The Heritage Foundation's project on tracking these cases shows 1,334 cases going back at least until 1983.
Since that time there have been 1,165,000,000 presidential votes cast.
Assuming at least that many in off year elections for all state and local governments, that's over 2 billion votes in the US.
If there have been 1,334 cases of election fraud ballots per their records. I would think that is very rare.
